Does this product have to be refrigerated?

It most definitely has to be refrigerated.

How do you pack this? How long will it survive outside the refrigerator?

It comes in a big, insulated, styrofoam box with cold packs. When I received mine it was late spring, but the contents were cold. I think, because each packet is individually sealed, it could tolerate a few hours of warm temperature

Can you explain the date codes on the boxes. I cant find any exp. dates on the cups.

I wasn't able to locate the exp date either. Sorry.
