Q: I'm looking for a pillow that will allow me to sit up in bed to read --not lounge or lie back at a 45 degree angle. Is this the right one?

I would say yes. You will be sitting at a 15-20 degree angle from upright but you will be virtually upright. Don't be concerned when it arrives severely compressed because it really expands when you follow directions. I am happy with mine. There is no headrest but I don't find that to be a problem. Q: Can this pillow provide support free standing or does it need to be learning against a wall or headboard?

This pillow is made with shredded memory foam and can be molded to provide support where you need. It can provide support free standing, especially for children and for adults when the foam is molded to match the way you are using it. Yes, my daughter uses her's in bed for tv or reading and really likes it ... She kept taking mine so I ordered one for her.
