Do you have ot use both when you open the two pack?

Two hand warmers come in a pack. They are activated by shaking them in the air. Consider immediately placing the other one in a zip lock bag and squeezing the excess air out. I have been successful doing this.

Are the hand warmers individually sealed in the 2 pack?

Yes!!! The hand warmers comes in pairs of 2. Each pair are individually sealed until opened.

Do they work in the toes?

The toe warmers are smaller, but if you have extra room in the toes of your boots I see no reason for the band warmers not to work.
