Q: I wear 8 1/2 shoe & ordered Women's 9/10. They are too long & too wide. I could trim length, but not width. I'm afraid the size 7/8 would be too wide. I don't understand the sizing.

I wear 8 1/2 shoes and I wear Spenco 7/8 and they're a perfect fit in my shoes, no need to trim. Also confirmed with a Spenco rep that it was the 7/8 I needed for size 8.5 women shoes I wear. The sizing chart is on their website as well and also says 7/8 for woman shoe size 8.5.

Q: Are these good insoles for someone with fallen arches and can they be used in athletic training tennis shoes?

Tes:I have tried every brand of insoles, have 20 pairs for all my various shoes, and these are the best I have found and reasonably priced. If I don't use insoles, I get plantar faciitis and these prevent it.

Q: Will these inserts work in women's flats?

They make inserts in women's sizes, or you can translate men's sizes down to women's.

Worthless

Don't buy it, trust me. I made the huge mistake of wasting 40£ for these worthless insoles. It's very overpriced and has none effect and there's no refund. Instead get some costom made insoles that are made specifically for your feet.
