How secure is the cap? Is it likely to fall off your keychain in your pocket or if your keys get tossed around?

The cap that pops the pen out is an O-ring. It comes out if you pull it but isn't likely to fall off in your pocket. I've had this pen on my keychain for years and it might have happened once. If your keys are tossed - again I doubt it would come off....if they are thrown or dropped from a distance it's possible.
