Can you substitute with CR123 batteries?

You can use CR123A as a backup. You certainly cannot charge the CR123A.

Would these work with surefires rechargable cr123a's?

The 123 are 17 mm in size were 16340 is 16 mm pretty close they might work but the volts for the 123 is 3.0 and 16340 is 3.7 volts. With this headlamp is very bright when you have it cranked all the way up I personally use 16340 and found them to be very good in cold weather.Hope that helps.
