The photo shows 4 coax connectors. i'm confused. i expect 3 con, 2 antennas, and one used for signal down and dc up. what is the 4th connector for?

One of the four is just a mold and doesn’t have the actual hookup ,on mine it is the third connector spot. But yes it has two input for dual antenna and the coax serves as DC current. Hope that helps somewhat.

Does this include one part that mounts on the mast, and a second part that plugs into an outlet inside the house on the other end of the coax?

Yes it has a mast mount for two inputs and another box at the bottom with an electrical plug and two port output for two tv’s.

Will the built in 5g lte filter also filter out 4g lte noise?

This is all confusing because people use the phrase 5G filters and 4G filters like they are different things. But this filter removes all transmissions above channel 36 - the top of the TV spectrum. It does not matter what those signals carry - 4G, 5G, remote controls, WiFi, whatever, they are all gone. So it will remove 4G and 5G and everything else. And leave the TV channels alone.
