Will this work with Midas or Beringer X32 routing?

This is a passive adapter. It has no bearing on your digital audio I/O or routing. It does not convert your analog channels to AES50 or any other digital standard.

With gender changers, can I run two mics one way and 2 returns the other? Or is it just one way for all 4?

I'm using them that way with a gender changer as a monitor send from the mixer. But like John C. said, there might be cross talk if you're sending a line level signal back with the mic level inputs. I'm running all my outputs as mic level to powered speakers to avoid that.

The seller claims it passes phantom power. A reviewer claims it does not. Well, does it or does it not carry phantom power?

Most, if not all CAT5/6 cable breakouts require a shielded (STP) cable. There aren't enough pins to carry 4 balanced channels + ground otherwise. The ground is required for phantom power to work.
