is a test card included?

No card not even a battery. The thing does work though. I plugged it in to the wall socket and it has been giving background radioactivity levels continuously for several months.

Is this thing supposed to make a beep or click like a classic geiger counter when it detects radiation? I hear nothing when sound is set to "open"

I got the device to detect radiation (from radium) on old watches I restore. This confirms I should put on a mask and gloves and deal with the radium paint safely. I do indeed hear a few clicks from radium dials, and an alarm will go off after 3-4 hours hours in a closed space from the watch as the miliverts build up. Alarm will also go off when background radiation measurement is saved to memory if left on over a couple days. But the device does not register clicks like an expensive geiger counter from normal background radiation. If trying to detect radiation from the soil or in a home, a better choice is a radon detector.
