does this device allow 5v or 3.3v on the input and output pins?

Definitely not 5 volt compliant. According to the Raspberry RP2040 chip spec the maximum digital input voltage is IOVDD +0.3 VOLTS. Since the maximum IOVDD is 3.63 volts, the absolute max would be 3.93 volts.

Does it come with pin connectors?

mine came with two sets of header pins which you must solder in place if you wish to use them.

Can you use this with accelerometer to make a spirit level?

Sure. You would also need a display and a battery. Or use a USB cable and display on a computer. But this would be overkill if it weren't so hard to get old cheap chips these days. It's fine for a one off, but if you're thinking of production, check out things like older PIC an AT-Tiny parts. But if it's an actual spirit level you want, just buy one. If this is for inside a vehicle or robot, beware that accelerometers can only detect level in an unaccelerated frame (such as stationary, or constant speed including no turning).

what board choice in Arduino for code uploading? Does not upload as Uno

In Auruino IDE, go to board selection, board manager and search for Seeed RP2040 boards. Download and install. Restart Arduino for it to recognize new libraries.
