does this work with sega saturn?

I don't think so, but you can use the Pound HDMI cable for Sega Saturn.

Is this a 9pin adapter?

It has the same pin layout as the sega genesis model 2 cable and an adapter for model 1

Is the audio reversed some of the hdmi cables out there have reverse stereo?

Audio is not reversed, however if you have a 32X upgrade...sound is still Genesis headphone jack only. If you have an older TV with A/V jacks, you can use a Y cable for sound from the TV. I used a small Bluetooth speaker with an IPod auxiliary cable. Will work either way.
