can this be used on a DIY battery pack? the longer the battery pack is used, the lower the voltage becomes. I need it to be a constant 12v output.

Don't forget it is a STEPUP converter. Therefore to adjust its output to 12V you need a lower voltage as input. That lower voltage needs to be able to deliver more amps than what you need as ouput. The limit output being specified at 4A. The output voltage will necessarly drop if your input source can't provide enough amps. Example: If you need an output of 12V with 1A, you might need 6V with 2A (a little more, in fact, because of the imperfect efficiency which transforms into heat) Hope this clarifies.. Mike

The input voltage shouldn't be a problem. It should be OK providing your TV is rated at less than 48W

No this unit takes low voltage 5volts DC and converts it to a voltage between 5 and 35v DC depending on the adjustment screw . If you connect it to wall AC it will blow up and catch fire

Mine keeps overheating (recorded over 100°c) when i run a 5v, 1a power supply to it and step it up to 13.8v, even with heat sinks added, why?

Difficult to say on so little information such as how much current you are drawing from it. If it is overheating with no load, check you have the polarity correct. If the power is connected the wrong way it will probably get hot. Also check you have input and output correct. If above is as it should be the unit is probably faulty. Check solder joints for dry-joints or solder bridges.
