Pics do not show 8 buttons, so how are there 8 programmable buttons?

The scroll wheel flicks left and right, and pushes down, which makes 3. The button below the scroll wheel is 4. The two buttons on the side are 5 & 6. And left click and right click make 7 & 8.

Do you have to use Synapse for custom button programming? I want an office mouse and can't install applications on my work computer. Thank you.

Razer Synapse is needed for the initial mapping, but the mouse will remember the last mapping you made, even without Razer Synapse(excluding macros.) If you have a personal computer, you could map it on that and it would remember the mapping when it connects to your work computer.

What wheel tilt will do under macOS?

The ProClick works with MacOS. You will not be able to download Razer Synapse which is the program which allows users to customize the mouse controls, but other than that, the product will work perfectly fine.
