Is it 2 scoops per serving or 4?

I think it is 4. My husband had throat cancer and the radiation treatments made it super dfficult to eat. This stuff was a life saver for him. Most people lose 50 or more pounds in treatment, he lost more like 20 or 25. 4 scoops was often too thick to swallow for him so we used less but he drank several a day.

Does this help you gain weight ?

Absolutely you can gain weight according to how many scoops you use. I don't need to gain weight so I only use one scoop. I use it to keep from loosing weight and for the nutrition it provides.

Does these taste better with Milk or Water????

Milk tastes better and smoother, but water is bearable and not chalky.

Are the ingredients in this considered safe and healthy ?

As a vegetarian nurse that needs extra calories and nutrition (we skip meals, hazard of the job) I would say so. I have been using this product for years. That being said, if you meet any of the criteria under the warning by all means heed them.
