I do a physical job and felt that I was becoming quite hunched and round shouldered. Reading other reviews about it being uncomfortable I thought, well surely it’s doing its job? I’m a size 14/16. I ordered a large. First time I put it on it was a bit tight, however, the item was supplied with an expander, if the item was too small, which I thought was a good call from the supplier. I managed to fasten mine ok and did not need mine. I do feel it is very comfortable and doesn’t hurt. My bra fits nicely over the top. While good quality the large was no where near big enough for my size 14, it was so tight under my boobs even after 10 mins I was struggling to breathe and was red raw. Perhaps that's why it rubbed at the arm as well, digging in painfully. I do find them impossible to wear with my normal (underwire) bras unless I wear them under the bra, but otherwise they're great. I bought the largest size, as I'm a 36, and it is a little tight, but I can still wear it for long periods, so it's not too bad.
