Is there a shorter strap available?** A: You can contact the seller to resolve this issue. **Q: What size for mens 48' waist?** A: Measure around your hips, not your waist. I found the size is pretty accurate but you do have to go by hip size. **Q: My hips circumference is 35.5". Should I purchase the medium (29-35") or the large (36-41")?** A: I am almost the same as your measurements and I chose the medium. It fit fine. I was concerned that the large would leave to much spare strap and be bulky and uncomfortable. My hernia was very low on my body. **Q: Is the skinny strap wht holds it in place?** A: The main waist band holds the pad where you want to place it. The skinny strap runs from the back of the waistband, under your crouch to the pad. This places more pressure (you adjust it to the amount you need) on the pad, and stabilizes it. I would have given it a 4 or 5 star rating. But, just as many other buyers reported, the Velcro attachment on the smaller (under crotch) strap stopped "sticking" after about two weeks. The Velcro attachment is undersized given the stress load and pull on this strap. Therefore, the 3 star rating. Also a bit frustrating that the company has done nothing to correct this design flaw despite many users reporting the same problem. I will add additional Velcro myself to get it to stick for another week!
