Even though I also wear a full face CPAP mask which covers my nose and mouth, if I forget to wear this collar overnight, my AHI (hypopneas per hour) are 15-18 (according to my CPAP machine's sensors), but if I wear this collar, AHI is only about 1-2. The more I tighten the collar, the lower the AHI. This collar keeps my jaw and mouth from gaping wide open, but nothing outside of a vise could completely prevent you from opening your jaw when you are asleep. Believe me, I've tried every style of collar, chin strap (including homemade), and even taping my mouth shut with medical tape. Try any of those things and you will see you can still open your jaw a bit especially if you tilt your head back a little. This collar does reduce it enough to make a big difference in apnea. I found a 4" collar too high (made me bend my head back a little and still wouldn't guarantee a closed mouth. The 3" collar height was right for me. Can be used any time of the day to help relieve pain , affordable. ### ⭐⭐⭐⭐ More comfortable than the hard collar! *by D***R on October 24, 2025* My neck measurement was 16 inches, and clavicle to mandible was three and a half inches so I ordered large as recommended on the sizing chart. There is still a very wide gap in the back, which does allow a lot of neck extension. The soft padded part of the brace itself does measure 19 and 1/2 in. The removable sleeve is nice for keeping the neck brace clean. I don't really think the plastic stiffener adds anything to the brace, but when I ordered it didn't cost any more to order the dual purpose brace anyways. I wore this after neck surgery, after I was allowed to stop wearing the hard cervical collar. It does not seem to limit neck extension very much if at all, but it does significantly limit flexion, and limits lateral bending to about 20 degrees. It does not limit rotation of the head at all.
