does it come with an applicator?

Not sure what applicator is. It comes with 3 labels for properly aligning the placement of the protector and one label for sticking up the screen dust for the last time before finally laying down protector. Also alcohol cleaning cloth for initial screen cleaning.

Does this work with the OtterBox case?

I have the otterbox commuter series case on the Pixel 5a and it works well. Comes a fraction short of the case so it fits nicely. No complaints.

does this fit on the phone screen fully?

The screen protector covers all but the camera circle and a very thin border around the phone. That thin gap at the edges makes it compatible with most cases. Buy a multipack because one of mine was giving me fits during install and I had to trash it. :(
