Good morning - not sure if this will help or confuse the question. The manufacturer uses measurements from the neck to the base of the tail. Labradors and boxers usually take a Large (26.4" 0 29.9"). Our 33lb. dog wore a Medium and it fit perfectly. Much, much, better than the cone alternative.

This was the second time my fur baby had major surgery. The first time I used a onesie but this time it did not cover the upper shoulder are as well as the abdomen. It was also too small and there was not a larger one. This suit was perfect as it was almost a turtleneck and came in different sizes - larger than a onesie. As with the onesie it had to be unsnapped for dog to relieve himself. I just unsnapped under tail and peeled it back to fully expose penis for dog to urinate. When peeling back just snap loosely around midsection or under loop inside under belly. and it will not get wet. I would have to assume this would work also as well for a female.
