How often do you recommend replacing this pillow?  I mean, the foam can't last forever, right?

This is a great neck pillow. I have had mine for 5 years and am just needing to replace it. 
It doesn’t break down like most pillows.

I have knots in my neck always. Would this foam make my neck more sore? Double Core or Tri Core Excel or Iso Cool so many choices?

The Tri Core Excel helped me for a while, but it broke down pretty quickly.  The quality of these pillows isn't such that they last long.  I eventually ended up with this Iso-cool contour pillow: http://www.amazon.com/gp/product/B000ZK7MPG/ref=oh_details_o00_s00_i00?ie=UTF8&psc=1  It isn't perfect, but it is the best I have found.

what is the foam made of?  Not the cores. The rest of the pillow. Is it polyurethane foam?

I found this on Core Website, Says Foam then explains about the polyurethane foam, so my guess would be polyurethane foam:A.  Fiber:  Allows for comfort and support.  High quality fiber is blown to exact specification to insure uniform distribution and pillow weight.
