For a full but not overstuffed pillow, what size insert should I buy for a 16x16 sham?

Dear Customer, For a nice plump pillow that is filled from corner to corner we recommend purchasing one size up. So if your pillow cover is 16 x 16 you must purchase an 18 x 18 pillow insert. If you'd like a more natural looking pillow you can go with a 16 x 16 pillow insert. Thank you

is it machine washable?

The big purple text of the ad says it is "machine washable". However, the "seller", Uban Mall, writes in this very Q&A section, that they "don't recommend using a washer and dryer". They recommend hand washing for "best results". I bought a lot of these pillows and I have no intention of spending all that time hand washing each one of them and laying them out forever to dry. That is a process that probably won't even probably clean all area of the entire pillow anyway - especially if you have the larger size pillows.

Do these make your 18 x 18 pillow fat? I'm looking for one that doesn't make a pillow look like a basketball.

No! I purchased 20 x 20 inch pillows to put in my 18 x 18 inch pillow covers and they are nice and fluffy looking, but definitely NOT FAT!! Hope this helps 😉
