No setup required. This device only needs to be within range of a cell tower. No service contract required because law mandates all cellular devices must be able to call 911 free.

Do you need to test the botton when you first get it?

Yes. Just push the button let them know you just received it and are testing it to be sure it is working

No carrier required it just calls 911

Will the 911 operator know your location if you are unable to speak?

I'm not sure. I think I recall mention of a GPS component. I do know the one I bought will do what I bought it for. My wife accidentally activated her 's and the 911 operator answered immediately.
