The title says this is a 4-pack, but several reviews imply there's only one refill in the package. Is it 4 or just 1?

If you look on the page, it has a drop down that says "size" if you click that option, you'll see where you can select an actual 4 pack, but I wouldn't recommend that, as a 4 pack is $96 and you could buy 4, individually, for around $30.

What prevents the ball from sliding out? I know the feather comes out which is fine but what about the ball?

The ball is attached to the hook. Never had any problem with that falling off the hook. But if your cat is rough, they're going to break the feathers off the ball. It doesn't last long enough for the price, but they do love it.
