how do you know what size to get?

There's a size chart in the second picture on the top left. Unfortunately for a lot of us it's in cm instead of inches. A lot of the US measuring tapes show cm so check that out. I wear a women's sized 9.5 athletic shoe and I bought a Medium, which fit well. Hope that helps.

Do you get two or is this just one brace?

One per package, you'll have to order two ankle support braces.

Does the strap length vary between the sizes? Or is it only the size of the actual "sock" part that varies?

Bigger the size longer the strap. It’s symmetrical. Your not going to get a strap so long that you can wrap around your foot/ankle several times. Good luck.
