It supports & protects your balls from shifting motion & torsion better than underwear, compression shorts or a traditional jock. Simpler, cooler, drier. The Teste Nesty is a must have when this kind of care is needed. It's lightweight, comfortable, and easy to clean. I would go 1 size up from what you think you need just for added comfort and/or if you need to put items like gauze in there.

Like most folks I got this after surgery for better support. Jockstraps push down, ball hammock underwear that cradles underneath is good, but the best support was this item. The waist band adjusts on the left side with a bit of extra fabric to pad the plastic. The pouch is stretchy, soft, and tight mesh. Above it is an elastic band. The pouch itself (I got the small) isn’t the support, it’s the area behind it that slips under your sack that gives the support. That’s a more rigid, but comfortable strip of fabric that doesn’t stretch. Seems like the theory behind under-wire bras, and the support and lack of pain while healing is excellent. It only feels tight when the waist band is not level; I had a tendency to hike it up which is totally unnecessary. The plastic clasp has been solid for me, I don’t change the size once fitted and am not wearing much over it as I’m home recuperating. Yeah, it costs a bit more than a jock, but the lift up at the right spot makes it worth it to me.

Small is not golf ball size. The sack is like cheese cloth and has a lot of stretch. Pool/tennis ball more like it. To get what feels like support it has to be pulled up tight to the point it starts to pull a bit too hard behind the boys, and it gets sore after a while. It worked but.. meh.
