Q: Can I use this with 6/3 UF wire?

I believe that 6/3 would have two hots a neutral and a ground. The plug is designed for two hots and a ground. So you will have the neutral wire left out. If this is for making an extension cord or some such it isn't a problem, just a superfluous wire. If it is directly out of an appliance, there may be a good reason for it to have both a neutral and a ground.

Q: To be certain, the "white" and "black" is hot and the "green" is ground?

That would be correct, when wiring for 220v the white and the black are both hot, and the ground is green.

Q: Does this plug have blades that will fit a NEMA 6-30R outlet. That is, both blades the same size and turned 90° to what is shown? Thank you.

Yes. It comes with a second blade that is the same size as the right hand blade in the picture. It is marked 30A. If you zoom in on the picture you can just make out the slots turned 90 degrees from the installed blades. I have two of these but I am using them in the 50 amp configuration.

These plugs are hard plastic - no rubber coating.
