What material is the wheel? Is it rubber? In the picture it looks like it is not made of rubber.

Please know the wheel is coated with rubber. It works for any wooden floor, carpet or other floor types. Thanks for your attention. Vinsguir Customer Satisfaction Team 100% Customer Satisfaction 24/7 Customer Service 30-Day Free Returns & 12-Month Warranty & Lifetime Maintenance

Does this work on carpets or blankets, or only on hard floor surfaces? Will that affect the training?

Both are OK, may be slightly rubbing on carpets when you roll it, and smoother on hard floor surfaces. Hope this helps. Vinsguir Customer Satisfaction Team Customer-Oriented: 24/7 Customer Service 12-Month Warranty & 30-Day Free Returns

Is this something you would recommend if someone has upper to lower back pain?

No, It requires a fair amount of back strength, both upper and lower to use. You could easily make it worse if you have an existing problem already.
