Q: is the water bottle holder big enough for a standard size bottle...? read somewhere that it could only take a small narrow 250 ml?

No. The bottle holder will easily take a 500ml bottle. The top is elasticated to keep the bottle in place but once opened wide it will accept the standard 500ml bottles. It is also removable from the Waist pack for when you don't want to take the entire pack but you need water you can put it on an ordinary belt.

Q: I want to use this for airsoft, will those ammo style pouches fit an M4 mag?

To the best of my knowledge these would be too small

Q: does this fit a hydro flask 18oz (532 ml)??

It would be a very tight fit. Not worth the effort.

Q: Is 45" max belt size including the pack or without the pack ?

The max belt size does not include the pack.
