Doers this one have the small projections at top and bottom that hold bags in place?

I have both two of this item. Once in a brick and mortar store and had the two small projections you are asking about. I had problems with them because some rolls of bags doesn't fit inside and other ones jam. Maybe that's the reason why manufacturer suppress that projections, but trust me, they are unnecessary. No matter how wide is the roll of bags due to use them, you won't have any issue with this items. This is made in China, the main item in Germany and I have been using it almost for 8 years in a row.

What size bags I need for the holder?

Any standard size bag should work. We use the Top Paw brand silver bags from PetSmart.

Will this fit the small flexi classic nylon cord retractable dog leash?

I have the black Flexi retractable leash which is either small or medium. Should fit either. Works best to hold doggie bags, opens at both ends. I first used to carry treats but the little rubber cap kept either dropping off or dropping inside. It has no cord so easy to lose.

There is no spot under the handle of my small flexi leash to fit this on

Maybe the small flexi leash doesn’t come with it? Mine is just a square opening and a slot to secure it into. Snaps in very securely..I love it
