a well layed out and well written text
This clean colourful and brilliantly layed out volume is an ideal student introduction to questions of patient safety and healthcare improvement. With over 50 contributors, this book covers a great deal in its 124 pagesStarting with the basics of patient safety and then understanding systems, there are a number of essential definitions and discussion points, such as organisational accidents and quality healthcare. There is a meaningful discussion of human factors, teamwork and communication and a sect ion on reporting and learning from errors.Sections on understanding and interpreting risk and the wide range of types of risks t patient care are also helpful. The book concludes with a wide section on quality improvement.Chapters are colour coded and enhanced by a rich range of illustrations, graphs, charts and tables. This book is an excellent introduction to the subject and would make the basis of a number of good essays around the general premise. It also gives any medical student or health care professional a welcome reader to the subject.Highly recommended!

Very good and way overdue.
I gave this book 5 stars BUT let me say this book is simply a re-worked theme other industries have been using for the last 30 years in earnest. That of Quality Control and Quality Assurance with some good old iterative improvement techniques thrown in for good measure. It's frankly well overdue. So the book itself:Well laid out in clear and decisive chapters:Part1: The Essence of Patient SafetyPart2: Understanding and interrpreting RiskPart3: Risks to patient care.Part 4: Quality Improvement. Each part of the book is filled with clear high quality diagrams, techniques and improvements. My wife is a healthcare professional and found a lot of this very interesting but as an IT professional who has worked in many industries I found most of it very familiar. The langauge is easily accessible and the concepts are exlained clearly. My only concern is having seen this before I know where this is going to lead. To yet another level of beauracracy with well intentioned people piling on even more hoops for medical staff to follow while trying to do their day jobs. As always this is a solution to a root problem that people won't address and is yet another elephant in the room that joins the growing herd.Definately worth a read.
