This book gives a lot of examples of how to do a life care plan for many disease/disabilities. Good resource.

This is a required text for an online class I am taking. I looked at a few other sites, (I won't mention) and found the best deal here. It's a great resource book and this edition is easy to read, not unnecessarily wordy. Thank you Amazon!

But the quality of writing varies considerably from chapter to chapter, and the scientific support (supporting references) are sorely lacking. A greater reliance on actual standards of care in medicine would serve this discipline greatly.

Well written book on the subject. It is comprehensive and provides good background as well as both conceptual and concrete information.
