Yes, you can strum with it. I don't finger pick so someone else could answer that question. I play a lot of mouth harp and it works there too. It is a soft sound, and hard to pick up even with a vocal mic, so don't expect a great big sound from it.

What’s this filled with?

This shaker is filled with small steel pellets.

Anyone already using it - does it not interfere with playing cajon? I normally take out my rigs to not make that sound and scratch the cajon..

it attaches to your finger with an elastic strap, therefore it cannot scratch the cajon. use it on the middle finger of your "lead" hand and it adds some nice shaker sound to the beat. it does not interfere with your playing, maybe just a tiny, tiny bit when you play at the very top edge of the cajon. i also have the similar aluminum finger jingles FJS1A-BK ($8 on amazon) which also attaches with an elastic strap. The finger jingles are lighter on the hand than the finger shaker.
