I found the power supply p/n ACX5126DCV at a site named 'coolgear' for $19.95.

Q: Is this warrantied? i bought mines on may 19th last year. i deduced the usb is not sending signal to my usb monitors anymore.

Q: Does this USB hub have protection to prevent it from back feeding power up to the computer when powered by the external power adapter?

I don't know if there is a built in protection for back feeding, but I have been using the hub for over a year with no known issues.

Q: Is a power supply included or not?

Yes, mine came with a power supply. I was impressed with the build quality. It has half dozen USB devices plugged in and is happy. It will mount on the back of my computer table, so it's much easier to unplug than my old plastic model.
