Is the DLC on the disc or is it codes to retrieve online?

It is the full game with DLC included on the disc. The main screen says THE FIRE FADES EDITION. As you may know those places won't be available until you are ready for them.

i hope to god this works with the regular dark souls 3 save data

It works!!! Tried it myself. However you cannot go back to regular dark souls 3 disc after using your save in this version.
