big head,, small head,, neck issues.. these will decide how comfortable this is. why ? well, you raise the head and neck slightly, when using this on a bed. if you have a true massage table, your head stays level, if not a slight downward angle. if you have any issue raising your head, or leaning your neck back, this might not be as comfortable as you wish otherwise, its close to 5 stars for comfort.. really nice.. only used sparingly, so cant speak to longevity..
