Can these be used in cold salads? How good are these ?

Yes you can, but no it won't taste good. Whatever plant flour they used (lupine flour I think it was), you will taste it. It does not matter how it is cooked or used, or how much sauce or cheese you use, you will taste the plant flour that it is made from. I know from personal experience, even others who tried it with me, agreed you taste a weird plant flavor.

Where is this product made? Why does it evade saying it on the bag?

According to their website, the pasta is made in the United States. If you need more specific info, try contacting the company directly.

Is the serving size for cooked or uncooked ?

The serving size is 1/4 the package. I cooked half the bag the first time (so 2 servings, video is half of what I made) and it swelled up excessively which ended up being way too much. So 1/4 the package dry will make a good portion for one person once cooked in my opinion.
